# Understanding program sustainability through Benefit-to-WAC ratio

[Claims Adjudication](https://www.infinityrx.com/news-insights/tag/claims-adjudication) [Gross-to-Net](https://www.infinityrx.com/news-insights/tag/gross-to-net) [Benefit Design](https://www.infinityrx.com/news-insights/tag/benefit-design)

Benefit-to-WAC ratio doesn't get the attention it deserves. At its simplest, it measures how much a manufacturer is spending in copay benefit relative to the WAC, or [Wholesale Acquisition Cost](https://www.kff.org/medicaid/5-key-facts-about-medicaid-prescription-drugs/), of the prescriptions flowing through the copay program.

Think of it as a pulse check on how hard those copay dollars are working, and what that can mean for the sustainability of the program and the broader gross-to-net picture. Put another way: for every dollar of product revenue flowing through the copay program, how much is being spent in benefits?

The metric doesn't tell you a manufacturer's true gross-to-net. Copay programs are one part of that picture. But it can provide useful context for understanding how efficiently manufacturer dollars are being used to support patient access. And sustainability matters because [you can't support patient access if a program is in the red](https://www.infinityrx.com/news-insights/program-leakage-a-closed-loop-framework-for-recovery-and-prevention).

## Key takeaways

- Benefit-to-WAC measures how much a manufacturer spends in copay benefit relative to the WAC of prescriptions flowing through the program.
- A lower ratio isn't automatically good news. View it alongside coverage rate to understand what's driving the change.
- The ratio tells you what happened. Coverage rate, payer mix, claim outcomes, and accumulator or maximizer activity help explain why.
- Claim-level visibility connects benefit-to-WAC with coverage and claim activity, showing whether the program is sustainable and efficient.

## A lower ratio isn't automatically bad news

A decreasing benefit-to-WAC ratio can be a positive sign, assuming volume is flat or increasing. Let's say a program's ratio falls from 58% to 46% over the course of a year. That sounds positive on its own, but the ratio alone doesn't tell you why it changed. Was payer coverage improving? Did utilization change? Did the mix of claims shift? Did something change in the program itself?

The answer becomes clearer when you view benefit-to-WAC alongside coverage rate. If coverage rate increased from 54% to 72% during that same period, the declining ratio may indicate that more patients are obtaining payer coverage and relying less on manufacturer-funded assistance. In that case, the manufacturer isn't simply spending less. The program is becoming more efficient, and that's an important distinction. A lower number by itself isn't the objective. Understanding what's driving it is.

The ratio tells you what happened. The other data helps explain why.

This is where benefit-to-WAC becomes more useful: look at it alongside coverage rate, payer mix, claim outcomes, and [accumulator](https://www.infinityrx.com/news-insights/the-accumulator-problem-how-manufacturer-dollars-get-diverted-every-year)or [maximizer](https://www.infinityrx.com/news-insights/maximizers-the-quieter-more-expensive-threat-to-your-copay-program) activity. Those data points give you context for changes in the ratio and help you identify whether the program is becoming more or less sustainable.

A metric is useful when it leads to a better question. If benefit-to-WAC improves, what changed? If it deteriorates, what's driving it? If coverage improves at the same time assistance spend shifts, what does that tell you about how the program is functioning? Those are the questions that turn a program metric into program intelligence.

## Visibility changes the conversation

The challenge is that the data needed to answer those questions usually sits across multiple partners. [Copay administrators, hubs, specialty pharmacies, claims processors, and other organizations each hold part of the picture](https://www.infinityrx.com/news-insights/the-vendors-of-vendors-problem-why-fragmented-stacks-cost-you-more-than-you-think), which makes it difficult to connect coverage, assistance spend, and claim activity into a single view.

We see a different picture because [our claims adjudication infrastructure gives us visibility](https://www.infinityrx.com/news-insights/unified-dual-benefit-why-pharmacy-and-medical-belong-on-one-platform) into the underlying claims data itself. That visibility makes it possible to connect benefit-to-WAC performance with the other signals that matter: coverage, claim activity, and [accumulator or maximizer activity](https://www.drugchannels.net/2026/02/copay-accumulators-and-maximizers-in.html). The value isn't in watching one number. It's in understanding what that number is telling you.

For manufacturers, that's a more useful way to think about copay performance: not simply how much the program is spending, but whether those dollars are supporting patient access in a sustainable, efficient way. That's the real pulse check.

## What's missing from your affordability program?

Benefit-to-WAC can give you a useful pulse check. The next question is what's driving the number. We can analyze your claim-level data across copay, pharmacy and medical benefits, and reimbursement activity to help uncover where your program may be losing efficiency, exposing gross-to-net, or creating opportunities to improve.
